Chat rescue form not working

I’m trying to enable “chat rescue”. The rescue message displays after 30 seconds even though I’ve set this property to 45 seconds in the settings.

But more pressing is that the contact form that is displayed as part of the rescue message doesn’t work. We’ve tried multiple computers, and it looks like there is a server error.
